Have you or your company had a workplace accident or incident and facing possible litigation? Has your place of work or company experienced a high incident rate? I offer a different approach at looking at the root cause(s) of an accident or incident. I provide a review of my findings regarding gaps or failures that may occur on either side of the spectrum for potential guidance  in determining where fault may lay and possible solutions to rectify the issues and cut down on incidents. I rely on my past training and personal management position skills to look at all aspects of any particular incident(s) finding those gaps that many others may not see. Key aspects included are a compiled list of potential areas for failure related to either side. I offer consultation services to review any reports and findings that have been investigated to date, provide the leg work in finding facts and discuss possible missed factors involved to determine if there is a possibility for litigation or settlement claims to be valid and/or continue to investigate the incident/accident to bring a possible conclusion to the root cause and possible fault.
